Spring Waiver Deadline: January 19, 2012
If you already have health insurance, provided by your home country or another sponsor, and it will cover you while you are in the United States, you may waive out of the mandatory IU health insurance plan. You must submit proof of your health insurance coverage to the OIS by doing the following:

Make sure your health insurance policy meets the requirements for health insurance at IU:
- $100,000 minimum benefit ($250,000 is recommended) or
- $50,000 coverage per Sickness or Illness (must include maternity/pregnancy coverage)
- $50,000 coverage per Accident or Injury
- $10,000 coverage for Medical Evacuation
- $7,500 coverage for Repatriation
- Deductible of not more than $500
- Co-insurance of not more than 25%
- Effective Coverage Dates
- Fall Semester: August 15 (or first day of classes) – December 31
- Spring/Summer: January 1 (or first day of classes) – August 14
- Log into iStart to access the Insurance Waiver Request e-form. You can access the e-form up to 30 days before the first day of classes (July 30, 2011). The deadline to submit a waiver is 10 days after the first day of classes (September 8, 2011), and the e-form will not be available after that date.
- You will be required to enter information about your health insurance policy (e.g., company name, company address and phone #, policy ID #, effective coverage dates, etc.) and confirm that it meets IU's requirements for health insurance.
**If your health insurance policy does not meet IU's requirements, your Insurance Waiver Request will be denied, and you will be required to pay for the mandatory IU health insurance plan.** - Click "Submit" when you complete the Insurance Waiver Request. You will receive an email (at your IU email account) when your request is either approved or denied.
Don't forget: You must submit an Insurance Waiver Request EVERY semester!
